Overview
The LHPC Institute and California Health Care Safety Net Institute (SNI) are pleased to invite you to a convening of local health plan and public health care system leaders.
Building on the success of last fall’s event, the LHPC Institute and SNI will host a second in-person convening focused on data exchange and alignment across systems and plans. Plan and system leaders will continue to share approaches for improving data quality and workflows, diving deeper into topics such as transitions of care and health information exchange.
Join us for this important convening where attendees will:
- Learn how plans and systems are working together to address data gaps leading to more accurate quality reporting and effective improvement efforts.
- Explore opportunities to move from measure-level analysis to a broader joint data strategy.
- Examine transitions of care quality measures (e.g., Follow-Up After Emergency Department for Substance Use) as a case study for strengthening data exchange between plans and systems.
- Identify opportunities to better leverage health information exchange (HIE) to improve data quality.
- Strengthen relationships with plan and system partners through joint planning efforts.
Who Should Attend:
Participants should have a decision-making role in efforts related to data and quality, and may include Chief Health Informatics Officers, Chief Medical Officers, Population Health Directors, Quality Directors and others.

SNI and LHPC will present opportunities for Medi-Cal managed care plans (MCP) and public health care systems (PHS) to continue their work together to improve data exchange and alignment, with a focus on transitions of care and health information exchange (HIE).
Team Time: Part 1 — Progress to Date
Members of the Plan-System Data Affinity Groups will share updates on their efforts to improve data exchange and alignment.
As a pair (aka “dyad”), MCP and PHS partners will reflect on their progress in improving data exchange since the October 2024 convening and the work ahead.
Sharing Data to Improve Transitions of Care
Ash Amarnath, MD, Chief Health Officer, SNI
SNI will provide an overview of policies and initiatives that impact transitions of care and discuss the current and future state of follow-up quality measures.
Peer Share: Approaches to Improving Follow-up Care After Discharge
Dyads will present different models of follow-up care (e.g., same-day post-discharge appointment, follow-up with assigned Primary Care Provider; follow-up with a provider outside the PHS), highlighting data workflows and solutions for each.
Roundtable Discussions: Advice from Peers
Participants will join mixed groups of MCP and PHS leaders, share their top 1-2 challenges in improving post-discharge follow-up care and receive advice from the group about how to overcome those challenges.

Team Time: Part 2 — Reflections on the Morning Session
Dyads will discuss what they learned during the morning session and identify next steps to improve care transitions after the convening.
Optimizing Health Information Exchange
Bradley Gilbert, MD, Consultant, Former CEO of Inland Empire Health Plan
LHPC will provide an overview of health information exchange (HIE) and the role it plays in facilitating data quality and reporting for MCPs and PHS.
Participants will discuss the primary use cases for HIEs for plans and systems, respectively.
Roundtable Discussions: Optimizing HIE
Participants will join mixed groups of MCP and PHS leaders and share strategies to leverage HIEs to improve data quality and reporting.
Dyads that leverage EPIC’s Care Everywhere as an HIE alternative will discuss how to optimize the Care Everywhere platform.
Team Time: Part 3 — Takeaways and Next Steps
Dyads will share what they learned throughout the day and consider implications for their own work. Teams will identify 2-3 actions they will take to improve data exchange after the convening.

IMPORTANT NOTE: Each invited plan and public health care system may elect to register up to four (4) participants per entity. Plans and public health care systems are strongly encouraged to coordinate attendance prior to registering.
There is no cost to attend, however registration is required.
Registration Deadline: Registration closes May 22 at 5:00 p.m. or once capacity is reached.
Registration Confirmation: Registrants will receive an email notification confirming their registration.
Program Materials: Presentation and other program materials will be made available electronically only and will be provided to confirmed registrants.
