Alameda Alliance for Health Partners with Community in New Health Home Pilot
(Alameda, Calif) – In December 2016, the Alameda Alliance for Health (Alliance) will introduce a Health Home Pilot (HHP) program to improve health outcomes in our community. Through this program, the Alliance will create new community partnerships and provide funding to local organizations that provide case management services, to serve the people in our community who are most in need.
The Alliance HPP program will connect people with services for issues that affect health outcomes, such as disease management, mental health, alcohol or drug misuse, as well as food and housing security. The Alliance will partner with local community-based organizations to provide these case management services.
“The Alliance mission, vision and values are all about making sure people in our community receive timely access to affordable care and services to improve their health outcomes,” said Alliance CEO Scott Coffin. “The HHP is the latest in a series of care management programs offered by the Alliance, and will help the Alliance better understand the needs of our members who are most in need, and improve the coordination of care by creating linkages to the services that our members need to help improve their health.”
The Alliance recognizes the important role that community partnerships have in improving health outcomes, and invites all community-based organizations interested in participating in the HHP to complete and submit a Request for Information (RFI).
